Advance Health Care Directives are legal documents in which a person specifies a family member, agent, or physician on what actions should be taken for their health if they are not longer able to make decisions themselves. Preparing these types of documents provide clear instruction to families and offers peace of mind for all. This workshop provides assistance in completing and notarizing advance directives.
