Oct 2017: District & Brain Injury Center Create Seamless Transitions
The Camarillo Health Care District is partnering with the Brain Injury Center of Ventura County, area hospitals, and CSU-Channel Islands to provide support for individuals with traumatic brain injury or dementia, and their caregivers. The goal of the project, funded by Dignity Health St. John’s Hospitals, is to create a seamless transition for individuals who are moving from acute-inpatient treatment to community living using evidence-based interventions such as medication management, caregiver support and education, health education and empowerment, and linkages to community resources.
