Nail Care Clinic

Foot care is an often overlooked component of overall health status. Without proper attention to the feet, walking stability can be compromised, and serious health issues can be overlooked.

The Camarillo Health Care District's Nail Care Clinic offers medically necessary fingernail and toenail care for clients who are permanently or temporarily unable to perform their own nail care. Nail Care Clinic clients often have a variety of health issues that limit their ability to perform their own nail care, including such issues as a bad back, unstable or weak hands, poor eyesight, and hip or knee replacements, to mention just a few.

Enroll in the Nail Care Clinic
A completed Nail Care Clinic Application Packet is necessary prior to your first appointment. You may conveniently print this packet from your computer, pick up a packet at the Nail Care Clinic, or request that one may be mailed to you.

This application process is required in order for your physician to sign the Statement of Medical Necessity, which verifies client qualification and ensures your safety. You may view these forms online and print them at home, but please call in advance for your first appointment at (805) 388-1952, extension 136. Please bring the completed and signed form to your first appointment.

This form provides the Nail Care Clinic with a brief history of you in order to ensure your satisfaction and meet your needs.

Service Waiver/Consent Form (click here to view and print)
This form is required and gives your consent to participate in the Nail Care Clinic services.

Physician's statement of Medical Necessity (click here to view and print)
This form certifies that you are unable to perform your own nail care due to physical condition, illness, and temporary or permanent disability.

Notice to Medical Professional (click here to view)
This form is sent to your physician by the Camarillo Health Care District and explains the services of the Nail Care Clinic. You do not need to print or complete this form.

Services
Nail Care Clinic service includes a foot bath, toenail and/or fingernail trimming, therapeutic massage of feet and ankles, and nail buffing by a licensed pedicurist.

Rates
Application Fee: $12.00 per client (one-time only fee)
Foot Care: $13.00 per visit per client ($12.00 per visit per client for Cam/Somis residents)
Hand Care: $4.00 per visit per client

Cancellation Policy
If you are unable to keep your appointment, kindly notify the Nail Care Clinic at least 24 hours in advance in order to avoid a $3.00 service charge (applied to next scheduled visit). For your convenience, the District's voice mail is available 24 hours a day, seven days a week at (805) 388-1952 x 136.

Transportation
Transportation services are available for District residents through the Care-A-Van program. To ensure your safety, the Camarillo Health Care District cannot transport electric wheelchairs, power chairs, scooters, or similar heavy mobility chairs. If you need wheelchair assistance, the Camarillo Health Care District will provide a wheelchair during your transportation. Please advise the scheduling office when you make your reservation that you will need District wheelchair assistance. Personal care attendants may ride at no charge.
